Data from both preclinical and clinical studies indicate an interaction between rTMS effects and sleep. Here we report about rTMS for the treatment of insomnia. METHODS Two patients with primary chronic insomnia were treated with rTMS. One patient received 15 sessions of 10 Hz rTMS over the left dorsolateral prefrontal cortex (DLPFC), the other patient received 15 sessions of 1 Hz rTMS over the right DLPFC. Sleep quality was assessed subjectively. Objective sleep analysis was performed by using a portable one channel electroencephalographic sleep screening system (QUISI). RESULTS rTMS was well tolerated. Both patients reported sleep improvement. This was reflected by a reduction of sleep latency and increase in total sleep time. Furthermore, increase of rapid eye movement (REM) sleep latency was observed. CONCLUSION These pilot data, even if very preliminary, support the notion that rTMS algorithms with known antidepressant effect are associated with a prolongation of REM sleep latency. Further controlled studies are necessary to investigate rTMS for the treatment of insomnia. |
