Safety, Tolerability, and Effectiveness of High Doses of Adjunctive Daily Left Prefrontal Repetitive Transcranial Magnetic Stimulation for Treatment-Resistant Depression in a Clinical Setting
Daily left prefrontal repetitive transcranial magnetic stimulation (rTMS) recently received Food and Drug Administration (FDA) approval for the treatment of depression and offers an alternative to traditional approaches. This approval was based on a study using 3000 stimuli per day (15,000 stimuli per week) in adults with unipolar depression not taking antidepressant medications. Several meta-analyses suggest a dose-response relationship with TMS. This study was carried out before US FDA approval to test the safety, tolerability, and effectiveness of adjunctive high-dose left prefrontal rTMS in a clinical setting with particular attention to safety of higher doses and potential interactions with antidepressant medications, speed of response, and effects on suicidality ...

Repetitive transcranial magnetic stimulation (rTMS) of the dorsolateral prefrontal cortex (DLPFC) during capsaicin-induced pain: modulatory effects on motor cortex excitability
Evidence by functional imaging studies suggests the role of left DLPFC in the inhibitory control of nociceptive transmission system. Pain exerts an inhibitory modulation on motor cortex, reducing MEP amplitude, while the effect of pain on motor intracortical excitability has not been studied so far. In the present study, we explored in healthy subjects the effect of capsaicin-induced pain and the modulatory influences of left DLPFC stimulation on motor corticospinal and intracortical excitability ...

Repetitive transcranial magnetic stimulation in the treatment of epilepsia partialis continua
Repetitive transcranial magnetic stimulation (rTMS) is a technique for noninvasive focal brain stimulation by which small intracranial electrical currents are generated by a fluctuating extracranial magnetic field. In clinical epilepsy, rTMS has been applied most often interictally to reduce seizure frequency. Less often, rTMS has been used to terminate ongoing seizures, as in instances of epilepsia partialis continua (EPC). Whether ictal rTMS is effective and safe in the treatment of EPC has not been extensively studied. Here, we describe our recent experience with rTMS in the treatment of EPC, as an early step toward evaluating the safety and efficacy of rTMS in the treatment of intractable ongoing focal seizures ...

Michael J. Fox Foundation Awards $2.2 Million to Four Clinical Trials for New Parkinson's Interventions
Alvaro Pascual-Leone, MD, PhD, of Harvard Medical School will test the potential of noninvasive repetitive transcranial magnetic stimulation (rTMS) to improve both motor and mood symptoms in Parkinson's disease. In October 2008, rTMS was approved by the FDA for the treatment of major depression outside the context of PD. It has also shown promise to alleviate the motor symptoms of Parkinson's. Dr. Pascual-Leone and colleagues, working at four North American sites, will enroll 160 Parkinson's patients whose motor symptoms are significant despite medication and who have been diagnosed with a depressive disorder. The researchers predict that rTMS will improve motor symptoms, depression, or both.
